Office Visit Fees
Our office visit fees are based on the complexity of your medical problem. You will find that the charge for your first visit is higher than for a routine follow-up visit. If you are being seen for a new problem or for a complication relating to an existing problem, you may be charged a higher fee for that visit because of additional examination and work-up needed to evaluate the circumstances.
We will bill your insurance company for diagnostic tests performed on the day of your visit.
Professional fees
We, along with our patients, are concerned about the rising cost of health care. It is our policy to charge only for services which are medically necessary. Only those tests which are needed to reach an appropriate diagnosis and course of treatment are performed.
Our professionals are specialists. Our fees reflect the additional education, experience, expertise, equipment and support staff needed to assess and treat complex ear, nose and throat problems.

Surgical fees
With insurance coverage:
For all surgeries performed, we will bill your insurance company. Each insurance plan has different reimbursement conditions (from full payment, deductibles, co-pays to percentage allowances). Ultimately the full payment responsibility is yours.
Without insurance coverage:
If you have no insurance for the surgery to be performed, a deposit is required. Please contact our billing specialists at (520) 296-2454.
Basic Payment Policy
Payment for service is due in full at the time the service is provided in our office. We accept cash, checks, Visa, and MasterCard.
Patients with Insurance
We bill most primary and secondary insurance carriers for you if the correct insurance card and referrals or authorization is provided to us. Copayments and deductibles are due at the time of service. If an insurance company does not pay for any reason, you will receive a statement, which is due and payable in full upon receipt. Provided on this website, in the Our Practice section under Accepted Insurance Plans, is a listing of the insurance companies that we currently participate with. If your insurance company is not listed, please contact our business office at (520) 296-2454 to see if we will file your insurance for you.
Medicare Patients
We accept assignment for Medicare. We will also bill most secondary insurances for you. All copayments and deductibles are due at the time service is provided.
Non-Covered Services
Any care not paid for by your insurance company will require payment in full at the time of service.
Appointment Policy
New patients and returning patients that have not been seen in 3 years are asked to fill out a health history questionnaire. All patients are required to fill out our patient data form every month. You may download these forms from this website. They are located in the Patient Forms section. We also require a copy of your insurance card at each appointment.
Children/Minors Policy
Patients under the age of 18 are required to be accompanied to their appointments by a parent or legal guardian. If a parent or guardian is unavailable, then the adult that is bringing the minor must bring a written note from the parent or guardian.
