As a courtesy, we will gladly submit a claim to your insurance company, provided we have a copy of your current insurance card and a referral authorization if required. Please note that while we always do our best to assist our patients in any way, if your health insurance is a HMO and requires that you obtain a referral, it is your responsibility to acquire this referral from your Primary care physician. It is not the responsibility of this office.
